A DBQ organizes what your clinician already knows

A Disability Benefits Questionnaire is a standardised VA medical form. It can record diagnosis, history, test results, treatment, symptoms, severity, and functional effect — in the structure the adjudicator reads.

A completed DBQ does not establish service connection. It documents the condition. Whether that condition is connected to service is a separate question that often needs a reasoned medical opinion, and the legal decision belongs to VA either way.

Find the right one in the complete public DBQ directory. Read the title before assuming it covers your condition: the general respiratory form explicitly excludes sleep apnea, which has its own.

Can a VA doctor complete one?

Yes, within limits. VHA Directive 1134(3) states that veterans may ask their primary care or specialty care provider to complete a DBQ for conditions which are already diagnosed and documented and for which the provider is treating the Veteran.

Three timings are contemplated: during a routine visit where there is time and the information is available, outside a visit, or at a separately scheduled appointment.

One exception is explicit. For mental health DBQs, the directive recommends that the treating provider not complete it, to protect the patient–provider relationship. That is a recommendation about one category — not a rule that VA providers never help with forms, and it should not be repeated as one. Read the directive.

How to ask

Send the current form and the relevant records ahead of the appointment rather than arriving with them. Point to the tests and treatment that bear on it. Ask whether they can complete the parts their own examination and records support.

A request is not an instruction. A clinician is not obliged to reach the conclusion you hope for, and asking them to is both unfair and counterproductive — an opinion that does not hold up helps nobody.

A completed DBQ also does not guarantee VA will skip a claim exam. Attend the one you are scheduled for.
